Terms and Conditions of the VIII Congress of the Polish Chamber of Insurance
1.1. The VIII Congress of the Polish Chamber of Insurance , hereinafter referred to as the “Congress”, shall be held on October 20-21, 2021 at the Sheraton Sopot Hotel.
1.2. The organizer of the Congress, for the Polish Chamber of Insurance, is Centrum Edukacji Ubezpieczeniowej Sp. z o. o. with its registered office in Warsaw at Twarda 18, 00-105 Warsaw.
1.3. These terms and conditions are an integral part of registration be for the Congress and are binding for all Congress participants.
1.4. The Congress shall include lectures and panel discussions.

§2. Congress Participation
2.1. The conditions of Congress Participation include:
2.1.1. online registration for the Congress through the Congress website www.kongres-piu.pl,
2.1.2. payment of the fee in the amount indicated by the Organizer by October 8, 2020, and receipt of Congress Participation confirmation (to the Participant’s e-mail address indicated at registration) from the Organizer.
2.2. The Congress fee should be paid to the bank account indicated by the Organizer. Information about the fees is also published at the Congress website www.kongres-piu.pl
2.3. The Organizer shall issue a VAT invoice for the payments made.
2.4. The Congress fee, accommodation and travel expenses shall be borne by the Congress Participant.
§3. Resignation Policy
3.1. A Congress Participant may resign from his/her participation in the Congress. Resignation form participation in the Congress should be made in writing and sent via mail or e-mail to the Organizer’s address.
3.2. Should a Participant resign from his/her participation in the Congress:
3.2.1. by October 1, 2021, the Organizer shall return the whole paid Conference fee amount to the Participant,
3.2.2. after October 1, 2021, the Organizer shall be entitled to retain the whole sum paid,
3.2.3. participation of another person in the place of the originally registered Participant shall be accepted by the Organizer.
4.1. The Congress participation fee is PLN 2600 including VAT for members of the Polish Chamber of Insurance, and PLN 4600 including VAT for other Congress participants. The fee does not include accommodation and transportation costs.
5.1. Any claims of Congress Participants to the Organizer should be made no later than 5 working days after the day of the end of the Congress, in writing via registered mail with receipt acknowledgement, to the address of the Organizer’s office or via e-mail: email@example.com.
5.2. After that date no claims shall be processed.
§6. Information about the use of the image
6.1. We inform that during the Congress Organizer or related parties will take photos and make video recordings for emission in mass media (television, radio, internet, press) or for Congress documentation and promotional purposes.
§7. Information about personal data processing
In accordance with article 13 sub article 1 and 2 of the General Data Protection Regulation dated 27th April 2016 relating to the protection of natural persons with regard to the processing of personal data and rules relating to the free movement of personal data, hereafter referred to as “GDPR”, Polish Insurance Association informs that:
7.1. Controller of personal data submitted in the Congress registration form (hereafter referred to as „personal data”) is Polish Insurance Association with its office at ul. 7.2. The purpose of personal data processing is commencement and execution of Congress participation agreement;
7.3. Personal data will be processed in accordance with article 6 sub article 1b) of GDPR in order to commence and execute the agreement or – before the agreement commencement – in accordance with article 6 sub article 1c) and 1f) of GDPR;
7.4. Personal data shall not be used for automated decision-making, including profiling;
7.5. In order to process personal data for the purposes indicated in point 2), Organiser entrusted the task of data processing (with a separate agreement) to third parties for registration and payment settlement purposes;
7.6. Personal data shall not be transferred to a third country;
7.7. Personal data will be stored for the period of 6 years for financial reporting purposes;
7.8. Congress participants have the right to request from the controller access to and rectification or erasure of personal data or restriction of processing concerning the data subject or to object to processing as well as the right to data portability. In order to execute these rights participant shall contact Polish Chamber of Insurance at: Twarda 18, 00-105 Warsaw, or e-mail address: email@example.com ;
7.9. Provision of personal data is a requirement necessary to enter into a contract and participate in the Congress. Failure to provide such data makes entry into congress participation contract and participation in the Congress impossible;
7.10. Congress participants have right to lodge a complaint with the President of the Personal Data Protection Office, Stawki 2, 00-193 Warsaw.
§8. Final provisions
8.1. If Congress does not take place for reasons attributable solely to the Organizer, the Organizer shall forthwith reimburse the sums paid in by the Participants to bank accounts indicated by them.
8.2. Submission of the registration form published in the Congress website constitutes acceptance of the Terms and Conditions of the Congress and observance of the regulations and any other arrangements made between the Participant and the Organizer.
8.3. To all matters not regulated by these Terms and Conditions, provisions of the Civil Code shall apply.
8.4. The Organizer shall not be held liable for the things owned by the Participants, which may be lost, destroyed or stolen during the Congress.
8.5. Participants assume full liability for damage done by them both on the premises, where any activities associated with the Congress are carried out, as well as in the places of accommodation.
8.6. Organiser preserves the right to amend Congress Terms and Conditions. After amendment is announced by the Organiser, every registered Congress participant shall acknowledge that amendment. In case the registered participant does not accept introduced changes to the Terms and Conditions of the Congress, he/she shall inform the Organiser thereof via email: firstname.lastname@example.org. Such information shall be submitted to the Organiser not later than 5 days after amendment’s entry into force. If the Organiser does not receive any such information, it shall be assumed that Participant accepts the amended Terms and Conditions.
